What are the next steps and immediate management goals in CGE?

Cerebral Gas embolism is a preventable medical emergency and all preventative steps should be put in place which are to:  

  1. Keep the patient flat
  2. Maintain oxygenation and provide haemodynamic support according to the ABC approach
  3. Early vasopressor/Ionotropic initiation
  4. Attempt aspiration if CVC in situ.
  5. Consider Durant position
  6. Stop or minimise further air entry immediately.
  7. Check all intra vascular access devices for their integrity, proper connections, valve function, air in lines and correct any defects 
  8. Notify the surgeons 
  9. Reduce the size of embolism by using the highest possible FiO2 
  10. Referral to the nearest Hyperbaric Oxygen therapy unit immediately
