What disease-modifying therapies are available for amyloidosis?

Treatment depends on the type of amyloidosis:

AL amyloidosis

  • Reducing production of light chains by targeting plasma cells
  • Regimens     include:     CyBorD                      (cyclophosphamide, bortezomib, dexamethasone)
  • Daratumumab-based regimens
  • Autologous    stem    cell    transplantation    in                      selected patients
  • Novel agents: venetoclax, isatuximab, IMiDs

ATTR amyloidosis

  • TTR stabilizers: tafamidis, diflunisal
  • TTR silencers: patisiran (RNAi), inotersen (antisense oligonucleotide)
  • Doxycycline for fibril disruption (investigational)
  • Liver transplantation (for hereditary variants)

Supportive therapy

  • Careful heart failure management
  • Anticoagulation for atrial arrhythmias (high stroke risk)
  • Prophylactic pacing for conduction disease
  • Dialysis for end-stage renal disease
