In the collapsed neonate the duct has closed needing a much higher dose to reopen it. It can be decreased once the duct is open to maintain patency. At such high doses apnoea and hypotension are common and intubation and ventilation is recommended. Other side effects might include flushing, pyrexia and vomiting.
