Key Learning Points

- Consider leptospirosis in febrile patients with jaundice and AKI especially if there is a history of freshwater exposure, animal contact or travel to the tropics.  

- Early antibiotic therapy is key with severe disease likely to require ICU support 

- Diagnosis is confirmed by serology or detection of Leptospira. but should be treated empirically if suspicion is high to avoid delay.
