- Optimal treatment of HLH is still being debated
- Current treatment regimens usually involve high dose corticosteroids for immunosuppressive effects, other therapies such as etopisode and cyclosporin can be used.
- Other treatment should be directed to underlying cause.
- Other medications used include Intravenous immunoglobins, methotrexate, vincristine and cytokine targeted therapy.
- Treatment is often aided with specialist input in a multidisciplinary fashion from infectious diseases teams, haematology and other specialists relevant to underlying pathology (eg; rheumatology).
In this case the patient was commenced on prednisolone prior to long term immunosuppressive planning.
Unfortunately, despite optimal medical therapy there was a gradual deterioration in the patients condition and the resulted in withdrawal of life sustaining treatment at day 15 in critical care.
