Case of the Month #44 High Anion Gap Metabolic Acidosis

What is the albumin correction for the anion gap?

Given that the AG depends on albumin (as the major unmeasured anion), an adjustment needs to be made for the albumin value, as every drop in albumin of 1g/L decreases the anion gap by 0.25 mmol/L.

The following formula can be used to correct this:

  • Corrected AG = AG + 0.25 x (40 – albumin (g/L))
