When would you consider stopping the Intravenous insulin infusion and restart subcutaneous insulin regime?
The subcutaneous regime can be restarted when the patient is able to take sufficient diet and fluids and blood ketones are <0.6 mmol/l and serum pH is >7.3
Subcutaneous regime should be restarted with a meal and the IV insulin infusion stopped 30–60 minutes after the subcutaneous dose
The diabetes specialist team should be involved in this transition
